Returning Clients

Hello again!

 

If you are an existing FoundationOne client OR have just set up an account and would like to order FoundationOne:

 

  1. Please download the test requisition form.
  2. Complete the test requisition form, including the information about the pathologist and institution where Foundation Medicine can retrieve the patient’s tumor specimen.
  3. Fax the FoundationOne requisition form, along with the pathology report and insurance information, to (617) 418-2290

 

  • Foundation Medicine will then complete the rest of the specimen retrieval process. We will contact the pathologist and send the pathologist a FoundationOne Specimen Collection Kit with instructions for sample procurement and shipment.
  • The FoundationOne report will be returned to the ordering physician within approximately 21 days from the date of receipt of the specimen at Foundation Medicine.

FoundationOne requires 40 μm of FFPE tissue from a block reviewed and approved by a pathologist as the most representative block for the patient diagnosis. All solid tumor types are acceptable. Please include the following items in the FoundationOne Specimen Kit prior to shipping to Foundation Medicine:

 

  • 40 μm tissue, of which ~20% is of malignant origin, on 8 to 10 unstained slides or from a block
  • FoundationOne Requisition Form
  • Copy of recent pathology/cytology reports
  • All solid tumor types are acceptable

Billing Process

Foundation Medicine accepts all insurance plans; however, we are currently not in-network providers with all insurance plans. Payment plans are available through our Patient Assistance Program, and assistance may be available based on the patient's financial situation. Foundation Medicine is committed to making the billing and reimbursement process as straightforward as possible.

When you order FoundationOne, we will work with you every step of the way: 

 

  1. A physician orders FoundationOne™ and provides complete insurance information on the test requisition form.

     

  2. Foundation Medicine performs the test and delivers the report to the physician. Results are typically returned to the physician within three weeks.

     

  3. Foundation Medicine bills the patient’s insurance company for FoundationOne, as the provider of service. Typically, it takes 60 to 90 days for the insurance company to respond to Foundation Medicine’s claims. During this time the patient may receive an Explanation of Benefits, this is not a bill and will explain the coverage initially offered by the insurance company for FoundationOne.

     

  4. If the insurance company denies coverage, Foundation Medicine works on behalf of the patient to attempt to obtain coverage and will assist in pursuing appeals on your behalf to minimize the financial burden when appropriate. We may also contact your office for assistance in the appeals process.

     

  5. Foundation Medicine receives payment from the insurance company. Depending on the terms of the patient’s health care plan (or in the case of an uninsured patient), there may be financial responsibility for payment of any applicable co-payments, co-insurance, or deductible for the test, or payment for non-covered services. Foundation Medicine will not bill for the difference between the insurance’s allowed amount and the list price of the FoundationOne test.

 

Foundation Medicine will work directly with payers and patients. Depending on the terms of patient’s health care plan (or in the case of an uninsured patient), patients may have financial responsibility for payment of any applicable co-payments, co-insurance, or deductible for the test, or payment for non-covered services. Please contact our Reimbursement Customer Service team for more information by calling (888) 988-3639 between 8:00 am ET and 6:00 pm ET, Monday through Friday, or send us an email.

 

 

 

 

 

Patient Billing Support

Foundation Medicine Offers Payment Plans

After a physician orders FoundationOne, the patient may receive an Explanation of Benefits from his or her insurance company - this is not a bill. Following the Explanation of Benefits, a patient may receive a bill from Foundation Medicine, in which case we will work with the patient to evaluate financial assistance programs and identify the appropriate next steps. There are no fees or interest charges for this program.

 

Financial Assistance May Be Available

Foundation Medicine may, in some cases, be able to provide financial assistance for patients who are uninsured or who have insurance and cannot afford the applicable out-of-pocket costs. Patients should contact our Reimbursement Customer Services Team with regards to eligibility.
